In case of an emergency, I authorize the camp staff to seek medical treatment for the participant, and I release the camp, its staff, and Reinhardt University from any liability. I understand the risks involved in participating in basketball activities and certify that the participant is in good health and can engage in physical activities.
I understand that the camp staff will take necessary precautions for the safety of the participants, but accidents may still occur. I agree not to hold the camp, its staff, or Reinhardt University responsible for any injuries or damages that may occur during the camp.
